New Patient Registration
We welcome new patients and we accept registrations if you live within the practice boundary. We encourage you to register as soon as you move into the area rather than wait until you are unwell, as records from your previous GP may take some time to be transferred to us.
You will need to fill in a form to register, and you will be asked the name and address of your previous GP so that your records can be traced. We also require sight of 2 forms of ID, one of which should be photographic ID. It is helpful if you can find out this information before you attend. Please come in to either of our surgeries to complete the paperwork.
When You Register
When you register you will be given information on the practice and our opening times. We will require 2 forms of ID of which one must be photographic ID.
You will be asked if you wish to register for our online services which includes some access to your GP record. You will also be invited to make an appointment for a general health check with the nurse. This enables us to record some basic general health information before your records arrive.
If you are on repeat medication, an appointment will be made for you to see a doctor. A review of your medication will be carried out and your details recorded so you can continue to request your repeat medication when it is due.
Our GP contract for 2015-2016 now states that we must allocate every patient with a named accountable GP.
In our practice this is the GP you were registered with when you joined the practice.
This is purely administrative and in no way prevents you from booking an appointment with another GP within the practice.
